We here at Southern Oregon Foot & Ankle grieve for our patients who have lost so much due to the wildfires. If you have lost your diabetic shoes due to the fires, we may be able to help.

If you meet the following criteria, please contact our office and let us see if we can help replace your shoes at no cost to you.

  • You are an established Patient at Southern Oregon Foot & Ankle.
  • You have received diabetic shoes by us within the past 2 years.
  • You have lost your diabetic shoes to the wildfires.

Our best wishes to each of you as you face these difficult times.

New Patients: Start Here!

We know that the first visit to a new office can be a little stressful for most people. We want every patient who sees us to be as comfortable as possible, and to know what they can expect.

Below, patients can access information they may need for an initial visit, including registration forms and general advice.

Our registration forms can be printed, filled out, and turned in as soon as you arrive at our office. This can save you time in the waiting room and get you into the exam room much faster. That’s the important part, after all!

We also provide some information on what you may expect from your first (and every) visit with us at Southern Oregon Foot & Ankle.

If you do not find answers to your questions here, please do not hesitate to give us a call at (541) 776-3338, or addressing our front office upon your arrival.
